Service Coordinator (SASH)

The Support & Services at Home (SASH) Coordinator works to build trusting relationships to develop a thorough knowledge of each SASH participant’s strengths and challenges as they pertain to living independently and safely at home. The SASH Coordinator regularly convenes the on-site SASH team to organize care and services to meet the needs of participants and further program goals.
Wellness Nurse (SASH)

The SASH Wellness Nurse provides and oversees wellness care and health coaching for SASH participants in accordance with the SASH Wellness Nurse Core Competencies, and coordinates health services with members of the SASH Team and other community health providers. The nurse supports a philosophy of aging-in-place and a participant-centered approach to care consistent with the mission of SASH. The Wellness Nurse advances wellness promotion, illness prevention, health preservation, and community-based nursing practices.
High Impact Tutor

NeighborWorks Blackstone River Valley (NWBRV) is seeking certified teachers or individuals with a degree in Mathematics or English to join our team as Tutors. The Tutors will be responsible for providing small-group, high-dose tutoring to support 10 high-need middle school students and 10 high-need high school students. The Site Coordinator at NWBRV’s afterschool program for middle school and high school students will work closely with schools and students’ teachers to identify students to connect with the high impact tutor, and ensure Multilingual Learners (MLLs) and students with Disabilities (DASs) are appropriately served through the out-of-school time tutoring program.
